Unexpectedly passed away in the comfort of his home on Friday, April 30, 2021, with family by his side, at the age of 54. Cherished son of Celestino. Loved by his brother, Carlos; sister-in-law, Fatima; niece, Kayla; godson, Dylan; and goddaughter, Samantha. Predeceased by his mother, Maria. Jack will be missed by many family members and friends near and far. A private visitation will be held at Westview Funeral Chapel, 709 Wonderland Road North, London. A private Funeral Mass will be celebrated at Holy Cross Santa Cruz, 10 Elm Street, London. A livestream of the visitation and Funeral Mass will also be provided for those wishing to view remotely.

Jack, there are no words for such a big loss. I will miss that jolly laugh and our conversations about all things tv shows and movies. On our drives home from work Jack always talked about his love for his family. I heard all about shopping trips with his niece, bringing donuts to his brothers on the weekend just to get some time in with them. He loved hard and will be missed dearly. Rest well Jack, and thank you for everything xox
We deeply sad with the passing of a great human being as Jack was. We keep the most vivid good memories of this long time close friend Jack Costa. Unfortunately we can not attend to the the funeral home or the church to pay our last respects. With sadness we extend our condolences to Celestino, Carlos and the rest of his great family. Jack and I went to school together, we lived on the same street a few years, our families were and continue to be friends...our friend Jack will be missed. Our lives took different paths but our friendship remained. R.I.P. dear friend.

He was the most down to earth, funniest and sweetest person that we have had the pleasure of calling our Primo. I have a lot of great memories of all of us as kids growing up together.
